So here’s something I’ve been thinking about.

In the flashback, Muzan was a dying human and received some experimental treatment from a doctor. That mysterious treatment ends up turning him into the first demon. But the anime or manga never explains how that doctor knew what he was doing.

Did the doctor already know about demons? Was he experimenting with some ancient potion or flower (like the Blue Spider Lily)? Or was it just an accident and Muzan became a demon purely by chance?

It feels strange that a random human doctor somehow made something that gave immortality, regeneration, and blood powers. Did he leave behind any notes or secrets? Is there any lore behind that doctor?

So, from what I understand, Muzan fell really sick as a young man. A doctor gave him an experimental medicine to try and save him. Muzan freaked out when his condition didn’t seem to improve and ended up killing the doctor in anger. But guess what? After that, the medicine started to actually work. It not only healed him but turned him into a demon, granting him immortality and regeneration, though with a catch: sunlight became deadly to him.

That’s where the Blue Spider Lily comes in. It was a rare ingredient in the medicine. Muzan only realized it was part of the drug after the doctor was already dead, and then began hunting for it for over a thousand years, but he never found it again.

So the doctor didn’t necessarily “know about demons.” He was just trying to treat a fatal illness with a radical, unfinished medicine that unleashed unintended effects. This accidental success created the first demon, Muzan Kibutsuji.
